HEALTH FORMS REMINDER!

(“Yellow” and “Blue” Forms)

 

Pursuant to Florida State Statute 232.032 The Heritage School must maintain current Immunization Record and Health Forms (D.C.F. Forms 3040 and 680) for all students enrolled.  No student will be allowed to attend school without current health forms. In addition, entering Level VII students must have a scoliosis screening on file as well.  Teachers will not admit to class any student whose documents are out-of-date on opening day.               

The Main Office will not call parents of students whose forms are not updated and on file by August 25th.  If you are unsure of your child’s health form status, please call the office before August 25th to avoid your child not being admitted to class.
